CDC Population Level Analysis and Community Estimates

What is it?

CDC's small-area estimates of chronic disease prevalence, health outcomes, prevention, and risk factors at the county, place, and Census-tract level. Built from BRFSS surveys with statistical modeling.

Why it matters

PLACES is the most-cited federal source for community-level chronic disease rates in CHNAs and grant applications. Over 30 measures including diabetes, asthma, COPD, depression, heart disease, and preventive care utilization.
